What if a patient needs help completing the Registration or Intake History process?

Due to reasons such as disability, age, illness or poor memory, some patients may need to rely on assistance from family, a friend, spouse or a care provider in order to complete "Steps" 1 and 2 of Registration (Step 1) and the Intake History (Step 2).

Steps 1 and 2 need to be completed at least 24 hours prior to the initial appointment time in order to avoid having the appointment rescheduled. Step 3, or the signing of Office Policies and Consent to Treatment, can be completed at check-in on the day of service.

The Online Intake History facilitates the gathering of personal, family and medical history needed in order to understand the many elements that can affect diagnostic accuracy and treatment considerations.

New Patients

You can begin the intake process from your computer by using the secure online intake process at Life Change Associates consisting of 3 simple steps.

Information that is submitted in Steps 1 & 2 is delivered to our office within minutes, through secure and encryption protected technology. Once received, the information in Steps 1 & 2 is reviewed by office staff as part of the initial registration process and you will then be contacted the same or following business day to schedule the initial evaluation.

New Online Registration

Step 1 -

Step 1 - Registration: consists of basic demographic information about the patient such as name, address, etc. and insurance information.

Step 2 -

Step 2 - Intake History: consists of medical, family and social history information that are needed by Dr. Ravsten as part of the initial evaluation. If you are 18 years old or older then click on the link below labeled Online Intake History.
